Iowa Medicaid Providers and Members are Invited the Community-Based Services Survey

The Iowa Department of Health and Human Services (HHS) and Mathematica have developed a survey to learn about people’s experiences with community-based Medicaid services. These are services and supports that Medicaid pays for to help people live, work and participate in their community. Examples of these services include:

  • Case management
  • Personal care and home health care
  • Medical equipment and assistive devices
  • Other services and supports

If you use community-based Medicaid services, are a caregiver (like a family member or natural support), a service provider, or a case manager – we want to hear from you!

The survey will be open until July 24. It will take about 15-25 minutes to complete. We will use the information gathered from this survey to inform ways that HHS can make it easier for people to access the services they need to be healthy and independent.

We want to hear from as many people as possible. Please share this survey with anyone who would like to provide input!
